What to bring

• Photo ID and any insurance cards (vision and medical)
• List of current medications and supplements
• Current glasses and any prescription sunglasses
• Most recent contact lens prescription or boxes
• Sunglasses for after dilation, if recommended

In-office paperwork

We'll provide a short intake form covering your medical history, current vision concerns, and consent for examination. Plan to arrive about 10 minutes early for your first visit.

Need to update something?

If your insurance, contact info, or medications have changed, just let our front desk know at check-in.
