Samuel Jacob Gouchenour Jr., O.D.

Dr. Jake is a 2025 graduate of the Kentucky College of Optometry. He blends a rigorous academic background with a passion for community-focused care. After earning his degree in Exercise Science from BYU in 2021 and obtaining Dean's List honors in medical school, Jake brings a modern, "whole-body" perspective to every eye exam.

He is dedicated to ensuring his patients leave the office feeling both well-informed and genuinely cared for.

When he's off the clock, Jake's life is defined by music and family. An accomplished musician, he is often found singing, performing, or playing the piano at his church and enjoys the challenge of mastering new instruments. While he loves hitting the ski slopes, his three children are his world, you'll find him spending time with them on his days off.
